To be eligible for any of the benefits listed here, you must be a full-time employee and have worked for the company, in good standing, for 90 days (528 hours+).

Health Insurance Policy

At Simonian Rugs, we care about your health and well-being. To help with your health care expenses, we provide every eligible employee with a $250 monthly contribution toward health insurance. This amount is paid out on the first paycheck of each month. This can update in the future as more programs become available.

How Our Health Care Contribution Works

  • Amount: $250 per month, paid with your first paycheck each month.
  • How to Use: Most employees use this contribution to help pay for health insurance purchased through Covered California.
  • Direct Deposit: Currently not available; the contribution is included with your paycheck.

How to Apply for Health Insurance (Covered California)

Most team members get their health insurance through Covered California, California’s official health insurance marketplace.

Open Enrollment:

  • Runs annually from November 1 through January 31.
  • You must enroll during this period unless you have a qualifying life event (such as losing coverage, getting married, or having a baby).

How to Apply:

  1. Online: Visit CoveredCA.com to apply online.
  2. By Phone: Call Covered California at 800-300-1506 for assistance.
  3. By Mail or Fax: Download the application from the website and submit it by mail or fax.
  4. In Person: Find free, confidential local help via the website.

What You’ll Need:

  • Social Security number or immigration documents
  • Proof of income (recent pay stubs or tax returns)
  • Information about your household and current coverage

COBRA Health Insurance Continuation Policy

At Simonian Rugs, we want to help you maintain health insurance coverage if you experience a qualifying life event—like job loss or a reduction in hours. COBRA (the Consolidated Omnibus Budget Reconciliation Act) is a federal law that allows you to continue your group health insurance for a limited time after your coverage ends.

What is COBRA?

COBRA lets you keep your employer-sponsored health insurance plan for up to 18 or 36 months (depending on the situation) after a qualifying event, such as:

  • Voluntary or involuntary job loss (except for gross misconduct)
  • Reduction in work hours
  • Divorce or legal separation
  • Death of the covered employee
  • A dependent child aging out of coverage (turning 26)

How to Apply for COBRA

  1. Notification:
    When your employment or coverage ends, Simonian Rugs (or our insurance administrator) will send you a COBRA election notice within 14 days.
  2. Review Your Options:
    The notice will include your rights, costs, and instructions for enrolling. Compare COBRA with other options like Covered California (coveredca.com).
  3. Enrollment Window:
    You have 60 days from the date of your COBRA notice (or the date coverage ends, whichever is later) to elect COBRA coverage.
  4. How to Enroll:
    • Complete the COBRA election form included in your packet.
    • Return the form to the address provided (can often be done by mail, fax, or online—check your notice for details).
    • Pay your first premium by the deadline. Coverage is retroactive to the date your group coverage ended.

Important Details

  • Cost:
    You pay the full premium (what you and Simonian Rugs paid before, plus a 2% administrative fee). This can range from $400–$700/month per person.
  • Coverage:
    COBRA coverage is identical to what you had as an active employee.
  • Duration:
    • Most qualifying events: up to 18 months
    • Certain circumstances (like disability): up to 29 months
    • Divorce or death: up to 36 months

Alternatives

Before enrolling, compare COBRA costs with plans on Covered California or the federal Health Insurance Marketplace (HealthCare.gov). Sometimes these may be more affordable.

Dental & Vision Insurance Policy

At Simonian Rugs, we want to make sure our team members have access to quality dental and vision care. That’s why we fully cover both plans for every eligible employee.

Dental Insurance

  • Coverage Amount:
    Each team member receives up to $2,000 per year for dental work. This benefit renews every January 1.
  • Network:
    We use the Principal insurance network for dental coverage.
  • How to Use:
    • Choose any dentist in the Principal network.
    • Present your insurance information at your appointment.
    • The plan covers a wide range of dental services up to your annual limit.

How to Find a Dentist

Vision Plan

  • Discounted Rates:
    Our vision plan provides discounted rates on glasses and contacts. The exact discount may vary depending on the provider and the products you choose.
  • How to Use:
    • Present your vision plan information at participating providers to receive discounts on exams, glasses, and contact lenses.

Important Details

  • Renewal:
    Dental benefits renew every January 1.
  • No Cost to You:
    Simonian Rugs pays the full premium for both dental and vision coverage.

Group Life Insurance

Group Life Insurance Policy

At Simonian Rugs, we are committed to supporting our team’s well-being and peace of mind. As part of our comprehensive benefits package, we provide group life insurance coverage to eligible employees—at no cost to you.

Eligibility

  • All team members are eligible for group life insurance after completing 90 days of employment.
  • Enrollment takes place during our annual open enrollment period in January.

Coverage Details

  • Benefit Amount: $50,000 of group life insurance coverage.
  • Cost: Simonian Rugs pays 100% of the premium—there is no cost to you.

How It Works

  • Once you have completed 90 days of employment, you will be automatically eligible for coverage during the next open enrollment period in January.
  • HR will provide information and any forms needed to enroll.
  • Your coverage will begin once enrollment is complete.

Workplace Injury Response & Reporting Policy

At Simonian Rugs, your health and safety are our top priority. We follow all state and federal laws—including OSHA and Cal/OSHA—when responding to and reporting workplace injuries. Here’s what you need to know and do if an injury occurs:


1. Immediate Steps if an Injury Happens

  • Ensure Safety:
    Stop work and make sure the injured person and others are safe from further harm.
  • First Aid:
    Administer first aid if you are trained, or call for help. For serious injuries, call 911 immediately.
  • Notify Supervisor:
    Report any injury—no matter how minor—to your supervisor right away.

2. Gathering Information

  • Document Details:
    Write down what happened, where, when, and who was involved. Include witnesses and take photos if safe to do so.
  • Injury/Illness Description:
    Note the type of injury (cut, burn, sprain, chemical exposure, etc.) and the affected body part.
  • Save Evidence:
    Keep any equipment or materials involved in the incident for review.

3. Reporting Requirements

  • Employee Reporting:
    • You must report your injury to your supervisor as soon as possible (California law requires notice within 30 days).
  • Supervisor/HR Reporting:
    • Serious injuries, illnesses, hospitalizations, amputations, or fatalities must be reported to Cal/OSHA immediately (within 8 hours for fatalities, 24 hours for serious injuries/illnesses).
    • All work-related injuries requiring medical treatment beyond first aid, resulting in days away from work, or restricted duty must be logged on OSHA Form 300 and reported per federal law.

4. Workers’ Compensation

  • Claim Process:
    • HR will provide you with a Workers’ Compensation Claim Form (DWC 1) and explain your rights.
    • Fill out and return the form as soon as possible to start your claim for medical care and wage replacement.

5. Best Practices for Carpet & Rug Cleaning Industry

  • Common Injuries:
    • Slips, trips, falls, chemical exposure, strains, and cuts.
  • Preventive Steps:
    • Wear proper PPE (gloves, masks, goggles).
    • Use caution on wet or uneven surfaces.
    • Follow chemical safety instructions.

Disability Policy

At Simonian Rugs, we are committed to supporting employees facing health challenges. We comply with all federal, California state, and local disability laws, including the Americans with Disabilities Act (ADA), the California Fair Employment and Housing Act (FEHA), and the California State Disability Insurance (SDI) program.

What Does “Claiming Disability” Mean?

Claiming disability means you are temporarily or permanently unable to work due to a non-work-related illness, injury, pregnancy, or medical condition. Disability benefits provide partial wage replacement while you’re unable to perform your job.


When Do You Qualify for Disability?

You may qualify for disability benefits if:

  • You are unable to do your regular or customary work for at least 8 days due to a physical or mental health condition, pregnancy, or childbirth.
  • You are employed or actively looking for work at the time your disability begins.
  • You have lost wages because of your disability.
  • You have earned at least $300 in wages (subject to SDI taxes) in the past 12–18 months.

How to Collect Disability Payments

California State Disability Insurance (SDI):

  1. Notify Your Supervisor & HR:
    • Let your manager and HR know as soon as you think you may need to file for disability.
  2. Apply for SDI:

    • Complete your portion of the claim and have your medical provider complete theirs.
    • Submit all paperwork promptly for faster processing.
  4. Payments:

Workers’ Compensation Policy

At Simonian Rugs, the safety and well-being of our team is our top priority. As required by California law, we provide Workers’ Compensation insurance through Farmers Insurance to protect employees who are injured or become ill due to work-related activities.

What is Workers’ Compensation?

Workers’ Compensation is a state-mandated insurance program that provides medical care, wage replacement, and other benefits if you are injured or become ill as a direct result of your job. This coverage is provided at no cost to you.

What Are We Required to Do by Law?

  • Maintain Coverage:
    We are required to carry Workers’ Compensation insurance for all employees, regardless of status (full-time, part-time, or temporary).
  • Display Notices:
    We post the official Workers’ Compensation notice in a visible location at our workplace.
  • Report Injuries Promptly:
    We must report all work-related injuries and illnesses to our insurance carrier (Farmers Insurance) and, in some cases, to state authorities.
  • Provide Claim Forms:
    We are required to give you a Workers’ Compensation Claim Form (DWC 1) within one working day of learning about your injury or illness.

What Should You Do If You’re Injured at Work?

  1. Report the Injury Immediately:
    Notify your supervisor or HR as soon as possible—no matter how minor the injury may seem.
  2. Get Medical Attention:
    For emergencies, call 911 right away. For non-emergencies, HR will guide you to an approved medical provider.
  3. Complete a Claim Form:
    • You will receive a Workers’ Compensation Claim Form (DWC 1) from HR.
    • Fill out the “Employee” section, describing your injury and how it happened.
    • Return the completed form to HR promptly.
  4. Employer’s Responsibilities:

What Are We Required to Do as an Employer?

  • Pay Unemployment Taxes:
    Simonian Rugs pays required state and federal unemployment taxes to fund the program.
  • Provide Information:
    When an employee separates from the company, we provide them with the required unemployment insurance information and promptly respond to any requests for information from the California Employment Development Department (EDD).
  • No Retaliation:
    Employees have the right to file for unemployment benefits without fear of retaliation or negative consequences.

Who is Eligible for Unemployment Benefits?

You may qualify for unemployment benefits if:

  • You are unemployed through no fault of your own (e.g., laid off, reduction in hours, or certain types of involuntary termination).
  • You are physically able and available to work.
  • You are actively seeking new employment.
  • You earned enough wages during the “base period” (usually the first four of the last five completed calendar quarters).

How to Apply for Unemployment Benefits

  1. File a Claim Promptly:

Eligible expenses may include, but are not limited to:

  • Other pre-approved business expenses

Note: All expenses must be reasonable, necessary, and directly related to your role at Simonian Rugs. If you’re unsure, please ask your supervisor or HR before making a purchase.

Best Practices for Expense Reimbursement

  • Pre-Approval:
    Whenever possible, get written approval from your supervisor or manager before incurring any business expense.
  • Documentation:
    Save all original receipts and invoices. Reimbursement requests without proper documentation may be denied.
  • Timeliness:
    Submit your reimbursement request as soon as possible—ideally within 30 days of incurring the expense.

How to Request Reimbursement

  1. Complete the Expense Report Form:
    • List each expense, date, amount, and business purpose.
    • Attach all receipts and supporting documents.
  2. Submit to Supervisor:
    • Give your completed form and receipts to your supervisor for review and approval.
  3. Approval & Processing:
    • Once approved, your request will be forwarded to accounting for processing.

When to Expect Reimbursement

  • Reimbursements are processed with the next regular payroll cycle after approval, or within 14 days of approval—whichever comes first.
  • You’ll receive reimbursement via the same method as your regular paycheck (currently by check).

  • Incomplete or late submissions may delay your reimbursement.
  • Expenses submitted more than 60 days after the date incurred may not be eligible for reimbursement unless there are extenuating circumstances.

What Discounts and Benefits Do Employees Receive?

  • Complimentary Rug Cleaning:
    Each employee is eligible for up to 250 square feet of rug cleaning at no charge per calendar year. This benefit renews every January 1.
  • 30% Discount:
    Enjoy a 30% discount on all Simonian Rugs services, products, and new rugs. This includes cleaning, repairs, protectants, and retail purchases.
  • Vortex Insulation Discount:
    Receive a 20% discount on any insulation services from Vortex Insulation, our sister company.

How to Use Your Discounts

  • Complimentary Cleaning:
    • Schedule your complimentary cleaning through our regular booking process.
    • Let the customer service team know you’re an employee when booking.
  • Discounts on Services & Products:
    • Present your employee ID or notify staff at the time of purchase or service request.
    • Discounts will be applied at checkout or invoicing.
  • Vortex Insulation Discount:
